USDA Non-discrimination Statement

The USDA non-discrimination statement for school nutrition programs is to be included on all printed materials that are distributed to students, parents, and the public. The statement below is the required language used on the DOE prototype on-line forms which are part of the division's Annual Agreement to Participate and is also required to be used on alternate forms approved by DOE.
